This week, Acacium Consulting Ltd is looking for a Health and Wellebing Coach based in Oakham. Here is a brief job description.

" We're looking for a Health & Wellbeing Coach to cover our Rutland contract, delivering the National Diabetes Prevention Programme within a 1:1 and group delivery setting. This will be to support service users who are both at high risk of Type II and Service Users who have been diagnosed with Type II Diabetes.

What you will be doing

Conducting both 1:1 and group delivery sessions to service users for your region

Maintain data monitoring records of all Service Users and ensure that this is handled with the greatest care

Consistently check and monitor the progress of the service users

From time to time, our Health & Wellbeing Coaches are required to carry out risk assessments."
