New Rutland Neighbourhood Health Team to open at new Oakham Transport Hub

Plans are being developed to create a Rutland neighbourhood health team at the new transport hub.
The former Jules House on Cold Overton Road in Oakham will become a new transport hub and at this week's health and wellbeing board meeting transformation officer at the Leicestershire Integrated Care Board Sammi Le Corre announced plans to site a new team on the top floor of the hub.

She said: "Towards the end of last year we were excitingly made aware we could use the top floor of Jules House for our hub purpose.
"Where we are at the moment is scoping and identifying what needs to go into that offer.
"We need something that is person facing and very accessible and available to anyone in Rutland who wants to see someone face to face for advice and guidance."
A neighbourhood health service is a key part of the Labour government's 10 year plan for the NHS and health boards across the country are devising new schemes.
The officer said there had been conversations between Rutland's health and care services and the local authority for many months about collaborating and discussions are taking place about what services could have a presence at the hub to create the new integrated team. Discussions have also been held with the ambulance service and DHU Healthcare which runs NHS 111 services across the Midlands.
Analysis has found that 1,237 of the 41,240 people registered with GPs in Rutland have multiple chronic conditions. Half of these have a care plan and half do not. There are 316 people in the county are identified as frail, with the majority having care plans.
The new neighbourhood integrated team would focus on these groups to help them access the suitable care.
The health transformation officer said work was expected to begin on the conversion of Jules House in May and for it to open in the early part of next year. She said the framework of how the different services would work together was being drawn up.
